(c) Program Registrant consents to their name, voice, and likeness being used by Good Night Sleep Site, Inc. for future lecture, teaching, and marketing materials, and further other goods/services provided by Good Night Sleep Site, Inc., without compensation to the Program Registrant or need for further agreement by Good Night Sleep Site, Inc.
(d) Required information includes (i) name, (ii) e-mail address, (iii) phone number, and (iv) city, state, and province may be collected by third-party services, ClickFunnels and ActiveCampaign, to collect contact information to distribute course material and information.
ONLINE CLIENT INTAKE FORMS
We collect your name, child’s name, street address, city, state, province, postal code, email address, and phone number in order to obtain the history and current sleep health, mental health, and physical health information of you or your child, in order to best provide services for you.
If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.
